IPCC Community Partnerships – 18 Partners, 20 Projects • Alzheimer’s Society – First Link Expansion • Canadian Mental Health Association – Counselling for Patients with Complex Health Issues • Pacific Post Partum Support Society – Strengthening Community-based Resources • Mood Disorders Association – Mental Health Wellness Support Program • BC Schizophrenia Society – BRIDGES education/support program • Squamish District – Exercise 4 Brain People with Mental Health Issues IPCC Community Partnershps High Need Community • Hailika’as Health – Enhancing Child Development and Learning for Health & Wellbeing Seniors People with Complex/ Chronic Conditions • Minoru Place Activity Centre, Richmond – Wellness Connections • North Vancouver Neighbourhood House – Peer Support; Golden Circle • North Shore Keep Well Society – Keep Well • Pemberton Valley Seniors Society – Bowling for Life • South Vancouver Neighbourhood House – Neighbourly Together • BC Lung Association – COPD-Related Educational Materials • Arthritis Society of BC – Expansion of Education Programs • Kensington Community Centre – Happy Heart-Chronic Disease Education • Langara YMCA – Access for All-Supported Health & Wellness • Powell River Employment Program Society – Humble & Hearty Workshop; Brunch-Education • North Vancouver Neighbourhood House – Volunteer Drivers 5

IPCC Community Partnerships Bowling for Life – Pemberton Valley Seniors Society Minds in Motion/First Link in Vancouver – Alzheimer’s Society Correct use of inhaler devices in 6 languages in video & written materials – BC Lung Association 6

Improvements & Lessons Learned  Identifying need of community 1. Client survey – resource intensive 2. Needs assessment reports – general recommendations 3. Consulting with care providers – quick; directly in touch with patients  Selecting the partners 1. Request for Proposal (RFP) – time consuming, low response 2. Targeted Approach – based on identified need; good response from potential partners 7

Improvements & Lessons Learned  Implementation 1. Flexibility with criteria – take in less complex; highly complex patients need more intensive support 2. For new program, at least a year to start up 3. Community partners are good with reporting progress  Integrating community partners with GPs and VCH community care providers 1. Informed GP practices; tracked referrals 2. VCH staff – face to face education session, info materials; tracked referrals 3. Community partner reported back to referring clinician 4. Participated in IPCC Steering Committee meeting 8

Improvements & Lessons Learned  Sustainability 1. Charge a fee 2. Fund with own funds 3. Help with evaluation to use positive results to explore other funders 9

Some Measures • 5,873 clients have received service • 60% of referrals come from specialists and GPs First Link • 50% of clients improved in their rating of overall health - Exercise 4 Brain and Neuro Health • 82% of clients improved in Get Up & Go Test Wellness Connections Program • 82% of participants felt workshops met their expectations - Arthritis symptom management workshop • 91% of clients improved in PHQ-9 scores for depression anxiety - Counselling for Patients with Complex Health Issues 10
