Neisseria Meningitidis (Meningococo)  Diplococo Gramnegativo  Intracelular o adyacente a PMNs  Presenta polisacárido capsular  Lipopolisacárido (LPS) en su pared  Hábitat natural: nasofaringe humana

Epidemiología  5-10 % de portación nasofaringea asintomática  Transmisión: Contacto directo (inhalación de gotitas de saliva)  Serogrupos A,B,C,Y y W-135  Grupos más afectados: - Niños ( 6 m - 3 años) - Adolescentes (14 – 20 años)

Patogenia Mucosa nasofaringea Adherencia a células epiteliales Invasión del compartimiento vascular Multiplicación y liberación de LPS Meningococcemia

Patología Infección del tracto respiratorio superior  Meningococcemia: Aguda leve Fulminante Crónica Meningitis Otras: Artritis, Neumonía, Endocarditis, Pericarditis

Meningococcemia Fulminante Sindrome de Waterhouse- Friderichsen 10 a 20 % de los casos 50 a 60 % de mortalidad

Meningococcemia Fulminante  Comienzo brusco y progresión rápida  Fiebre alta y escalofríos violentos  Petequias y lesiones purpúricas en piel, mucosas, músculo y suprarrenales  Shock, CID y falla multiorgánica  Depresión miocárdica y oliguria  Meningitis sin signos específicos

LABORATORIO  Leucocitosis con neutrofilia; neutropenia en casos fulminantes  Trombocitopenia  TP y TPT alargados  Fibrinógeno bajo  Productos de degradación del Fibrinógeno  Acidosis metabólica

Diagnóstico microbiológico  Muestras: Sangre, LCR, aspirado petequial, sinovial,pleural, pericárdico  Gram y cultivo   Contrainmunoelectroforesis PCR
