CBC or other preliminary tests : Dengue Suspect 4. Further Confirmatory Tests along with clinical symptoms : Probable 5. NS-1/ IGM/ IGG / PCR Positive Confirmed case Route of report: - Direct On line entry at Dash Board from Hospital - Report by fax or telephone from hospitals where Dash board Facility not available - Daily analysis and alert Generation at IPH (team of experts) - Preventive action at EDO/DO/DDOH level Disease surveillance Procedure & Protocols Slide7: Vector surveillance Larval surveillance (Indoor & out door) Female Teams (LSPs) deputed for door to door house hold surveillance. Male teams (SPs) deputed for out door surveillance Microplans develop Standard tools developed and in practice Online reporting system of all positive spots through android phones Activity continued throughout the year with two peaks . February to Mid April . August to September Surveillance Systems and control program Larva Surveillance (EQUIPMENT NEEDED): Larva Surveillance ( EQUIPMENT NEEDED) Ladle/ Dipper/ Larval nets Pipettes Larval collection vials Torch Labels (Time, date, Place) Magnifying lens Binocular microscope Formats – Daily return, Summary returns Slide9: Sampling strategies The sample size : calculated using statistical methods based on the expected level of infestation and the desired level of confidence in the results. Systematic sampling Every nth house is examined throughout a community or along linear transects through the community. Simple random sampling The houses to be examined are obtained from a table of random numbers (found in statistical text books or from a calculator or computer generated list). Laborious, detailed house maps or lists- prerequisite Stratified random sampling stratification based on identified risk factors, e.g. Un piped water supply areas / Poor sanitation services/ densely populated areas. A simple random sample is taken from each stratum, with the number of houses inspected being in proportion to the number of houses in that sector. LARVAL COLLECTION METHODS: LARVAL COLLECTION METHODS Dipping Netting Pipetting Whirl Pool & Siphoning LARVAL SURVEY - STEPS: 1. Examine all indoor and outdoor breeding sites / water bodies/ containers 2. Collect at least 10 Aedes larvae and pupae from all positive positive containers. 3. Place them in a labeled container (Date, Time, Number, Place, Collector name) 4. Fill the form simultaneously 5. Make further identification in the lab LARVAL SURVEY - STEPS Larva indices: Larva indices Container Index ( proportion of +ve containers) CI = No. of positive containers X100 / Total No. of containers examined House Index (proportion of +ve houses) HI = No. of positive Houses X 100/ Total No. of houses examined Breteau Index (relationship between +ve containers and houses) BI= No. of positive containers X 100 / Total No. of houses examined ADULT MOSQUITO SURVEYS (not recommended for routine surveillance): ADULT MOSQUITO SURVEYS (not recommended for routine surveillance) Indicators- landing/biting rate – landing-biting counts per man hour Indoor resting density (Hand / back pack aspirators) – Number of adult mosquitoes per house Number of adult mosquitoes collected per man per hour. Surveillance by Ovi-traps (Different designs): Surveillance by Ovi-traps (Different designs) Example: Ovitrap : 200-250 ml black plastic container 6 - 7 cm diameter 10.0 cm in height wooden ovi-position paddle wrapped in filter paper Fill up with water to ~ 80% full Place at every 100m distance Monitoring every 3 rd for presence of eggs or larvae
