What is excess mortality? • a premature death OR • one that occurs before the average life expectancy for a person of a particular demographic category Source:

The Problem • “In 2002, blacks suffered 40.5 percent more deaths (83,570 deaths) than would be expected if they had experienced the mortality rate of whites” • A decade later today, the total number of excess deaths is close to a million. There are huge racial disparities between blacks and whites in health care.

There are huge racial disparities between blacks and whites in health care. So how should these disparities be removed?

The Solution is Medical Homes! • also known as the patient- centered medical home (PCMH) • More medical homes should be built across the U.S to eliminate racial disparities and lower the excess mortality of blacks

Why Medical Homes? • Studies have shown that health care disparities are due mostly to where minorities receive treatment rather than to racism or cultural insensitivity. • Medical homes directly solve the problem of racial disparities by eliminating them.
